Nancy Dykstra, RN,MA,APRN,BC  Executive Director

As the key leader for the Health Center, Dykstra is committed to expanding health access to care. A nurse for 36 years, Dykstra is board certified as a clinical nurse specialist in public health nursing. Dykstra obtained her BA and MA from Augustana College in Sioux Falls, SD. Dykstra worked as a staff nurse for the University of Iowa Hospitals and Clinics, as a nursing instructor for Iowa Lakes Community College, as a staff nurse for Hope Haven RTC, and as a staff nurse for Sioux Center Hospital. Dykstra was instrumental in developing the homecare and hospice programs for Sioux Center Hospital and helped to reorganize Public Health for Sioux County. Dykstra is committed to initiatives that will impact the health and wholeness of the community.

Beth Strub, ARNP  Nurse Practitioner

Beth Strub has been providing medical services as a Woman’s/family health Provider since the Health Center opened. Beth and her husband live in Le Mars. She is a mother of 4 children. Beth values the relationships with her patients and is happy that many families in our community now have a medical home.

Belinda Lassen, CNM  Certified Nurse Midwife

Belinda Lassen has been a practicing midwife in Sioux Center . Over the course of 20 plus year,  she has delivered an increasing percentage of Sioux County’s babies. Under Lassen’s care, the Health Center’s prenatal numbers continue to increase and the entry into care indicator continues to improve. Belinda and her husband live in Sioux Center. She is a mother of 5 and a grandmother of 2.

Angelica Perez graduated from Sioux Center High School in the spring of 2008, and is currently attending Northwestern college pursuing a BA in Graphic Design with a minor in Spanish. Angelica provides intake, interpreting, and accounts receivable services.

Ruth Potter was born and raised in Sioux Center.  She graduated from ISU with a degree in Spanish. Ruth provides interpreting and advocacy services. Ruth also assists with inventory and purchasing needs at the Center.

Martha was born in Nueva Rosita Coahuila, MX and has been living in the US for over 18 years. While studying in Mexico to become a social worker, Martha got married and had 2 daughters before moving to the United States, shortly after her move she had another daughter making her a mother of 3.  Martha is responsible for dental scheduling, assists with rooming dental patients, interpreting for dental patients and support services for the hygienist. 

   

Jeanne graduated from the Methodist School of Nursing in Sioux City, IA in 1962. Jeanne worked in OB-GYN in Virginia, worked nights in the Sioux Center Hospital. Jeanne then worked at Mid-Sioux Opp. For WIC for a total of 27 years. Jeanne is married and has 5 children. She joined the health center shortly after its opening. Jeanne provides patient care and coordinates referrals and health center community services with patients and families.

Dr. Delywn “Del” Lassen, M.D.  Medical Director/Family Physician

Dr. Lassen earned his M.D. Degree from the University of Iowa in 1977 and completed a 3 year family practice residency in Sioux City.  While engaged in private family practice he served as a preceptor to medical students and family practice residents.  Lassen accepted a faculty appointment at the Medical  University of South Carolina in 1986 and later returned to Iowa where he serves as a clinical assistant professor for the University of Iowa College of Medicine as he supervises physician residents in the Siouxland family practice residency program. Dr. Lassen has been recognized locally and statewide for his commitment to students and his passion for family practice medicine.

Dr. Ken. Addink, DDS     Dentist

Dr. Ken Addink is a local dentist with a heart for missions, who, along with his wife, are key in developing out Dentistry program. Dr. Addink began seeing patients in November 2010. Addink is a graduate of the University of Iowa and has practiced for over 35 years.

Jolene Hofman,  Receptionist/Interpreter

Jolene Hofman is currently majoring in Social Work with a minor in Spanish at Dordt College in Sioux Center, Iowa. Jolene spent eight years of her childhood in Bolivia, South America, where she learned Spanish while her parents worked there as missionaries. Jolene works as interpreter and receptionist. She also manages the inventory for the Iowa Prescription Drug Program.

Pam has been a certified nurse midwife for seven years, and a registered nurse for 20 years. She is trained as a sexual assault nurse examiner. Pam holds a PHD in nursing and has served as the director of the nursing program at Dordt college for the past eight years.

Carolyn Strub, RN Clinical Nurse

Nelson graduated from Boyden-Hull High School in 2007. He is fluent in both English and Spanish and currently lives in Hull, IA. Nelson interprets at the Health Center for general office visits and Health Center events.

Barb is a licensed dental assistant. She, along with her husband Ken have provided dental care in the community for many years. For the past several years, the Addinks provided a free monthly clinic in Sioux Center and at the Sioux City Gospel Mission. Prior to becoming a dental assistant, Barb was a registered nurse (St. Gospel School of Nursing 1972). Barb and Ken have been married for 36 years and have 4 biological children and 12 adoptive children.

Roleen is a Nurse Practitioner who practiced 26 years in rural northern MN (Leech Lake Reservation) first as a public health nurse and the last 15 years as a Nurse Practitioner. In addition Roleen has served as Assistant Clinical Director to a team of advanced practice providers. She initiated a tobacco cessation program, rotated call as a sexual assault examiner and initiated the Reach Out and Read Program at the Case Lake Hospital. Roleen’s experience includes clinical work and administration. Roleen currently resides in Hospers.

Carolyn is a graduate of Western Iowa Tech (2010). She also has a Masters of Science in Exercise Physiology and a Bachelors of Science in Dietetics. Carolyn’s role at the Center is to assist in providing patient care, to provide patient teaching, and health and wellness services. Carolyn currently lives in Le Mars.

Joe Heitritter, Operations/Office Director

Joe Heitritter graduated from Northwestern College in 2008 with a degree in Sociology and a minor in Cultural Studies. After college, Joe spent a year in Paraguay with the Peace Corps. Joe currently lives in Rock Valley with his wife, Kendra and daughter, Lucy. Joe is responsible for leading day to day operations of GSCHC to ensure smooth work flow and patient satisfaction. He works closely with the non-provider staff as coach and mentor in the daily management of their programs and duties.

Brittany is a graduate of Northwestern College (2011).  She joined the health center team in May 2011, to help provide patient care and coordinate referrals and health center community services to our patients and families.  She is married and enjoys having the opportunity to serve the patients within the clinic.

Laura Aguilar is a graduate of Indiana University with a degree in Spanish and a minor in Sociology. Laura worked at SOAM in Martin, Tennessee before moving to Iowa. Laura works at Greater Sioux as receptionist and assists with interpreting for patients. Laura is married and enjoys spending time with their three children.
