General Medical Services

¨ Services include: Well-child care, immunizations,  treatment for childhood illnesses,  acute infections, women's health, chronic diseases, minor accidents and injuries.  Medical providers include a family practice physician and two nurse practitioners.

 

Prenatal Care

¨ Our Certified Nurse Midwives provide comprehensive prenatal care. Their combined years of experience are an asset to health center patients and families.

 

Dental Care

¨ Preventive and restorative oral health services were added in November 2010. Dental providers include a dentist, dental hygienist and dental assistant.

 

Chronic Health Care

¨ Greater Sioux Community Health Center Staff coordinate patient care with patients and families so that conditions such as cardiovascular disease,  chronic respiratory disease, and diabetes can be managed, and patients and families can have optimum quality of life

 

Iowa PDDR Participant

¨ GSCHC patients that need medication assistance may receive medications for a $4.00 dispensing fee through this state program. The purpose of this program is to improve the health of low income Iowans by authorizing medical facilities to re-dispense prescription drugs and supplies that would otherwise be destroyed.

 

Preventive Health Care

¨ Health Center patients are encouraged to adopt good health habits and to take measures that will prevent disease and injury. GSCHC believes that when patients understand risk factors and understand and recognize early disease warning signs they can reduce the disease burden on self and the community.

 

Lab Tests and Radiology

¨ GSCHC performs many patient lab tests on site in a CLIA waived lab. For those that can not be performed on site, the results are available, in most cases, the next day. Radiology services are available through contract agreements.

 

Women’s Health

¨ Women’s health services are met through a team of skilled Health Professionals. Services include: Pap Smears, Colposcopy, Family Planning, Counseling, and other services.

 

Children and Teen Health

¨ Our team of health care providers are dedicated to treating the entire family. Services include: well child care, vaccinations and care for a range of childhood illnesses.

 

Men’s Health

¨ Men are encouraged to seek regular medical care and to get regular physicals. GSCHC provides DOT physicals, workplace injury evaluations, and all other medical care services.

 

Immunizations

¨ GSCHC provide vaccinations for children of ages 0 to 18. GSCHC also provides a range of private vaccines.

 

Senior Health

¨ GSCHC recognizes the unique health needs of seniors. GSCHC assists seniors by providing affordable medical care and medication assistance and vaccinations.

 

Smoking Cessation Program

¨ GSCHC offers support to any individuals who wish to quit smoking or have a nicotine addiction. Please contact the health center for more information or to set up an appointment.

 

Free STD Testing

¨ GSCHC is one of 65 designated sites for the Iowa Department of Public Health’s Sexually Transmitted Diseases Program. Patients can be tested and treated for sexually transmitted diseases. Call 712-722-1700 for more information.

 

Free Colorectal Screening

¨ GSCHC participates in the Iowa Get Screened program and offers FREE screening to eligible 50-69 year old adults who are under-insured (have either no coverage for colorectal screening or need assistance paying the co-pay or deductible), are uninsured, or who are in households with income up to 250% of FPG (Federal Poverty Guidelines). Call 712-722-1700 to learn more about the screening.

 

Mental Health Services

¨ GSCHC partners with Seasons Center for Community Mental Health in providing mental health services at our center. Please contact the Seasons Center (1800-252-5101) to schedule an appointment.

 

Iowa Care For Yourself Program

¨ GSCHC will assist eligible women to enroll in the Care for Yourself Program. This program provides funding to low income women for breast and cervical cancer screening, which includes an office visit, mammogram and, if eligible, a PAP testing.
