The vision of a local community health center in Sioux County started in 2001. Grassroots efforts led by Public Health staff and the Community Action Agency (Mid Sioux Opportunity) Staff focused on exploring gaps in health care services to Northwest Iowa’s underserved. By August 2006 it was time to make the vision of a local community Health Center a reality. The Greater Sioux Community Health Center (GSCHC) was formally incorporated with a proposed location in Sioux Center. This steering committee submitted a grant to the U.S. Department of Health and Human Services. The project received Iowa Department of Health funding in 2007. The Greater Sioux Community Health Center is located at 338 1st Ave. NW in Sioux Center. Who we are GSCHC is a 501 (c)(3) non-profit organization. The center became a Federally Qualified Look-a-Like Health Center in 2009, and serves all residents of NW Iowa using a sliding fee scale based on household income and size. Our mission To increase access to comprehensive and affordable primary health care services for all residents of the greater Sioux county area. |
