About Us — Unity Through Diversity

The vision of a local community health center in Sioux County started in 2001.  By August 2006 it was time to make the vision a reality.  The Greater Sioux Community Health Center (GSCHC) was formally incorporated with a proposed location in Sioux Center.

A 20 member steering committee along with a grant writing team, put in motion a plan to apply for the U.S. Department of Health and Human Services grant.  The project was awarded Iowa Department of Health funding in 2007 and is seeking local match funding.  The health Center is located at 338 1st Ave. NW in Sioux Center in a newly remodeled facility.

Who we are

A full service medical clinic for all residents of NW Iowa that charges using a sliding fee scale based on income.

Our mission

To increase access to comprehensive and affordable primary health care services to all residents of the greater Sioux County area.
