Community Health Center Goals

-Provide preventative health services to keep    people healthy (at work, in school, etc.)

-Reach people who do not currently have a medical home

-Be a medical home for the medically underserved

-Provide chronic disease management

-Reduce inappropriate emergency room use.

-Reduce local clinic and hospital bad debt.
